PTO Request Form - Submit all PTO requests electronically for Charlie to approve.
Facility Reservation Form - Submit to use any school common areas. Times are limited to one hour for classroom parties.
Sublist 2024-2025 (PDF) - Remember, the responsibility rests with the teacher to contact subs if out for any reason.
Maintenance Request Form - Submit all classroom maintenance requests electronically to Mr. Kuhens.
Parent & Guardian Permission Slip - Permission slips need to be filled out and sent home 2-3 weeks prior to field trips. All permission slips must be completed on both sides and returned. When you return from your trip, turn in all permission slips to Bridget.
Field Trip Request Form - Step one, complete the Field Trip Request Form and turn it in for administrative approval.
Field Trip Car Assignment Form (PDF) - Prior to your field trip, sort all children into cars and give a copy of your groups to Bridget for approval. Note: All drivers must be cleared by the Diocese.
Terrific Kid Form - Each month, students in grades 1-5 are nominated by the teacher for Terrific Kid, a program sponsored by Kiwanis. The ceremony is held on the third Thursday of each month at 7:45 in the Library and families are welcome to attend. The program focuses on students who exhibit exemplary character traits such as kindness and empathy. Please nominate your student in advance by completing the form. Parents are notified the Tuesday before via email. Students should write a brief paragraph on why they are a Terrific Kid. Note: Teachers do not need to write a full paragraph, just remarks on why the student has been nominated that month.
Media Release 2024-2025 (excel) - This designates students who can or cannot be posted internally/externally and on all social media platforms.
